Missouri SenioRx Program

The state prescription drug program, named Missouri SenioRx Program, is in the sign up stage at this time. Applications for this program are now available at our center and we will assist you if needed.

Remember

You can only sign up for this program During April & May

We have the applications to hand out.

We will help fill them out and copy documentation if you need that service.

Monday & Wednesday 9-11am & 2-4pm.

Are you 65 or older as of July 1, 2002?

Yes go on to the next? No, do not apply, you are not eligible.

A resident of Missouri since July 1, 2001?

Yes go on to next? No, do not apply, you are not eligible.

Total household income is $17,000. or less for an individual?

Total household income is $23,000. or less for a married couple?

Yes go to the next? No, do not apply, you are not eligible.

Do you receive pharmacy benefits thru MEDICAID or the VETERANS administration?

Yes do not apply, you are not eligible.   No , apply you may qualify.

Do you have prescription drug insurance from employer or a medigap policy?

Yes this may render you ineligible if benefits are equivalent to Missouri SenioRx Program.

No Apply you may qualify.

Bring documentation of:

Age: drivers license or birth certification or Mo. ID

Residency: driver license, Mo ID , completed & signed income tax return with label for name and address, cert of residency in nursing home, utility bill  with name and address.

Income: 2001 income taxes, circuit breaker, copies of all income sources.

Dollar amount of 2001 prescription cost.

Information on other prescription drug insurance.
